A white card with black offset printing which was made available individually, as part of Kosugi's Events box and (inside a folded piece of paper with a footprint) in most copies of Fluxus 1.
Kosugi is performing at the Whitney on September 12 and 13th. Tickets went on sale yesterday, and can be purchased here.
